OUR DIDD SERVICES
PERSONAL ASSISTANCE (PA)
Our Personal Assistance service provides dedicated support to individuals living in their own homes. Trained staff members visit for scheduled hours each day, offering personalized assistance while respecting the individual's independence and comfort in their own space.
SUPPORTED LIVING (SL)
Our Supported Living program provides around-the-clock care, 24 hours a day, 7 days a week, delivered by trained and compassionate caregivers. This service is designed to ensure that every Person Supported receives consistent, attentive care in a stable and nurturing environment.
FAMILY MODEL PROVIDERÂ (FMP)
Our Family Model Provider program offers full-time, 24/7 care within the home of a trained caregiver. Persons Supported are welcomed as part of the family and share everyday experiences such as family dinners, vacations, holidays, and all the moments that make life meaningful.
COMMUNITY PARTICIPATION
We believe that it is important to let our Persons Supported participate in a healthy environment and be a part of the community. We take our Persons Supported to parks, community centers, and many events that our agency provides to integrate them into the community. All of these activities are structured under the guidance of healthcare professionals, to let our Persons Supported experience their lives to the fullest.
24 HOURS CARE SERVICES
We understand that care doesn't follow a schedule. That's why we offer professional, reliable, and fully personalized around-the-clock care services. No matter the hour, our dedicated caregivers are always available to provide the attentive, compassionate support that every Person Supported deserves.
VARIOUS ACTIVITIES
At All Heart, we make sure our individuals stay social and connected. We provide many activities such as bowling parties, prom, lunches parties, art and craft sessions, and holiday receptions. We structure these opportunities to ensure that our Persons Supported get to socialize and make friends with others.

All Heart Health Care Ministry is proud to offer a comprehensive range of Employment Community First (ECF) services, designed to empower individuals with disabilities to live, work, and thrive in their communities.
