OUR SERVICES

We provide compassionate, personalized care designed to help adults with intellectual and developmental disabilities thrive in their homes and communities. Understanding state programs can be confusing, so we have broken down our services by the specific Tennessee waiver programs we accept.

Our contracted MCO's include Amerigroup, Blue Cross Blue Shield, and United Healthcare.

DDA Services

Administered through the Department of Intellectual and Developmental Disabilities (DIDD), these traditional waiver services focus on long-term, deeply individualized support systems.

Personal Assistance

Direct, one-on-one support with daily living activities right in the person's home or community. Includes help with bathing, dressing, meal preparation, and grocery shopping to promote independence.

Supported Living

Support for individuals living in their own homes or apartments. Staff provides around-the-clock or scheduled drop-in assistance to ensure safety while preserving autonomy.

Family Model

A beautiful alternative to traditional care where the individual lives in the private home of a trained external family. This model fosters deep bonds and offers a structured, loving family environment.

ECF CHOICES Services

Employment and Community First (ECF) CHOICES is a TennCare program specifically focused on helping individuals integrate into their communities and explore independent living paths.

Community Living Supports (CLS)

A comprehensive service that helps individuals live independently. Staff assists with everything from household management and budgeting to medication reminders and social outings.

CLS - Family Model (CLS-FM)

Similar to standard CLS, but delivered within a family-style living arrangement. The individual lives with a caregiver's family, blending professional support with a natural, stable home life.

Personal Assistance (PA)

Targeted, hands-on help with activities of daily living (ADLs). This service bridges the gap, ensuring the individual can manage their daily routine safely and with dignity.

Supportive Home Care (SHC)

Designed for individuals living with their natural families. Provides critical respite for primary family caregivers while ensuring the individual receives meaningful, goal-oriented support.

CHOICES Services

The traditional TennCare CHOICES program focuses primarily on elderly adults and adults with physical disabilities, offering robust support to prevent nursing home placement.

Community Living Supports (CLS)

Provides the essential, everyday scaffolding—like personal care, light housekeeping, and meal support—needed for an adult to safely maintain their own residence rather than moving into a facility.

CLS - Family Model (CLS-FM)

Offers the exact same robust CLS care and oversight, but beautifully packaged within a shared, supportive family home environment to combat isolation and maximize safety.
