At All Heart Health Care Ministry, Inc., we provide a wide range of home health care services to individuals and families across Middle and East Tennessee. Our dedicated staff goes above and beyond to deliver personalized, high-quality care to every Person Supported because we believe everyone deserves the resources and support they need to live their best life.

Beyond daily care, we also foster a strong sense of community by hosting enriching activities such as themed parties, holiday dinners, and much more to create opportunities for our clients and staff to connect, celebrate, and thrive together.
OUR STORY
All Heart Health Care Ministry, Inc. was founded on March 15, 2015 by Chanthanome and Keo Thepmanivong with a heart for serving others. After a determined start, we welcomed our first Person Supported in October 2016. Just four months later, we opened our first Supported Living home in February 2017, followed by our first Family Model Home in January 2018. God has blessed us every step of the way, and we are grateful for each and every person we have had the privilege to serve.

As the organization continued to grow, Chanthanome and Keo passed the torch to their children, Nina and Nino, in early 2021. Under their leadership, All Heart Health Care Ministry has continued to flourish by expanding our services to include the Employment Community First (ECF) program, a dedicated branch focused on empowering individuals with disabilities to find meaningful employment and thrive in their communities.

From our humble beginnings to where we stand today, we remain rooted in faith, family, and a deep commitment to those we serve.

We are a healthcare provider that serves the intellectually, physically, and developmentally disabled populations to best support them in their local communities. We compassionately serve and empower our Persons’ Supported to live a humane, enriched, and vibrant life through our individualized care while also standing as advocates in the community through acceptance, integration, and unity.
